I usually start with a base of 2 parts ketchup and 1 part apple cider vinegar.  Here is the base recipe:

2 cups ketchup
1 cup cider vinegar
1/2 to 1 cup of brown sugar (add until you like the sweetness flavor)
2 tablespoons yellow mustard
1 tablespoon soy
1 tablespoon tabasco or red hot sauce
1 tablespoon woestershire
1 teaspoon red pepper flake
1 teaspoon black pepper
1 teaspoon salt
1/2 cup of your favorite store-bought bbq sauce (I use Stubbs spicy, you can use a sweeter if you like)

Heat in a sauce pan until the sugar dissolves, it's done.  Enjoy.

Sometimes it is easier to buy a good commercial brand you like, and doctor it up. If you want to go commercial; though I find it too sweet, Sweet Baby Ray's original is very sweet and thick. When I use it I often cut it down with some cider vinegar. Its easy to doctor up a commercial sauce to your particular taste. If it is not spicy enough, add some hot sauce. I've begun adding Tabasco Chipotle sauce to a commercial brand, with very good results.
